In Her Words…

“She’s great. She’s only been around for six months, and my kids are already looking for her. It almost feels like she’s part of our family. It feels good to have somebody around who wants to see me do better and recognizes that I’m trying to do better for my family.”
- Program Participant

“She was very supportive. I could call her any time. She advocated for me when I couldn’t advocate for myself, so it gives you more confidence. Just makes you feel more secure. Being pregnant is a hard thing, it’s a scary thing. To have someone by my side, I never felt alone.”

- Program Participant

“The support was phenomenal. I would recommend that others get in touch with Pathways – I have recommended others as well. I want to say thank you. I really appreciate it. You made a big difference in my pregnancy. “

  • The HUB Model

Starting in 2021, more than 12 local and regional organizations across the healthcare, public health, and social service sectors, explored the PCHI® model and jointly decided to bring a Pathways Community HUB to Omaha. Their vision was to explore how this innovative model could improve outcomes and create a better system of care for our community. The Omaha HUB officially launched services in 2024, focused on serving pregnant and postpartum women and their families. With the current goal of improved birth outcomes and healthier families, the HUB will aim to expand services for other groups experiencing barriers to health in the future.

Following the Pathways Community HUB Institute® model, the certified Omaha HUB is developing a sustainable network of Community Health Workers — trained frontline public health workers who have a deep understanding of and connection to the community. The HUB contracts with agencies that employ Community Health Workers, who connect with community members experiencing barriers to medical, behavioral, and social care. The CHWs partner with program participants to remove these barriers, improve access to care, and achieve their health and wellness goals.

In Her Words…

“She was, like, guiding me, setting me up, because I had told her, you know, hey, I found a place! You know, everything was good, but she was, like, giving me resources, people who could help, sending referrals. I wanted to get back into school, so she was helping me with that…”

- Program Participant

“I decided to work with Pathways and Arnetta while I was pregnant. She helped with diapers resources. I was having housing issues, so she was sending things about new housing and different things like that. She has solutions when I was having problems, so she is a problem solver.”

- Program Participant

“I can say my life has definitely improved. I am a lot better…..it’s definitely improved how I move around in life.”
